Abstract

Latar Belakang : Nyeri persalinan merupakan pengalaman subjektif yang dapat terjadi akibat kontraksi rahim, pelebaran dan penipisan serviks dan penurunan janin selama persalinan. Kontraksi rahim yang tidak efisien dapat membuat ibu mengalami kelelahan, ketakutan, dan stress. Nyeri ibu saat bersalin yang tidak ditangani dengan baik dapat menyebabkan persalinan lama dan meningkatkan risiko komplikasi seperti infeksi intrapartum, rahim pecah, retraksi cincin patologis, pembentukan fistula, cedera jaringan otot panggul pada ibu, dan ancaman terjadinya succedaneum dan molase kepala pada bayi. Salah satu metode non farmakologi yang dapat dilakukan bidan untuk mengurangi nyeri persalinan adalah kombinasi latihan counter pressure dan birthball selama kontraksi sehingga dapat mengurangi sensasi nyeri transmisi impuls ke otak
Tujuan Penelitian : untuk mengetahui efektivitas masase counter pressure dengan birthball terhadap penurunan tingkat nyeri pada ibu bersalin kala I fase aktif di PMB Wilayah Kerja Puskesmas Bukit Hindu Kota Palangka Raya.
Metode Penelitian : Jenis penelitian yang digunakan adalah pre- experiment dengan desain penelitian one group pretest-posttest without control group design. Metode pengambilan sampel dengan teknik purposive sampling, total sampel 29 responden.
Hasil Penelitian : Rata-rata tingkat nyeri persalinan sebelum diberikan intervensi adalah 6,24. Sedangkan rata-rata tingkat nyeri sesudah diberikan intervensi adalah 3,97. Hasil ini menunjukkan terdapat penurunan tingkat nyeri persalinan sebelum dan sesudah dengan selisih 2,27. Hasil analisis dengan Uji Wilcoxon menunjukkan p-value 0.000 yang artinya lebih kecil dari 0,05 yang artinya ada perbedaan nyeri persalinan sebelum dan sesudah dilakukan counter pressure dengan birthball.
Kesimpulan : Ada perbedaan tingkat nyeri sebelum dan sesudah pemberian counter pressure dengan birthball terhadap penurunan tingkat nyeri persalinan kala 1 fase aktif di PMB Wilayah Kerja Puskesmas Bukit Hindu Kota Palangka Raya.

Background : Labor pain is a subjective experience that can occur as a result of uterine contractions, dilation and effacement of the cervix and descent of the fetus during labour. Inefficient uterine contractions can make a mother experience fatigue, fear, and stress. Maternal pain during childbirth that is not handled properly can cause prolonged labor and increase the risk of complications such as intrapartum infection, uterine rupture, pathological ring retraction, fistula formation, pelvic muscle tissue injury in the mother, and the threat of succedaneum and molasses head in the baby. One of the non-pharmacological methods that can be used by midwives to reduce labor pain is a combination of counter pressure exercises and birth balls during contractions so as to reduce the sensation of pain transmitting impulses to the brain.
Objectives : To determine the effectiveness of counter pressure massage with birthball on reducing pain levels in mothers during the first active phase of labor at the Midwifery Independent Practice Working Area of the Bukit Hindu Health Center, Palangka Raya City.
Methods : The type of research design used was pre-experimental with a one group pretest-posttest without a control group design. Sampling method with purposive sampling technique, a total sample of 29 respondents.
Results : The average level of labor pain before intervention was given was 6.24. Meanwhile, the average pain level after intervention was 3.97. These results indicate that there is a decrease in the level of labor pain before and after with a difference of 2.27. The results of the analysis using the Wilcoxon test showed a p-value of 0.000, which means it is less than 0.05, which means that there is a difference in labor pain before and after counter pressure with a birthball.
Conclusion : There is a difference in the level of pain before and after giving counter pressure with a birthball to a decrease in the level of labor pain during the first active phase at the PMB Working Area of the Bukit Hindu Health Center, Palangka Raya City.
