Abstract

Latar Belakang: Stroke hemoragik dapat menyebabkan gangguan mobilitas fisik sehingga diperlukan asuhan keperawatan untuk membantu meningkatkan kemampuan bergerak pasien. Tujuan: Mendeskripsikan pelaksanaan asuhan keperawatan pada pasien stroke hemoragik dengan gangguan mobilitas fisik. Metode: Penelitian ini merupakan penelitian terapan dengan pendekatan asuhan keperawatan pada pasien stroke hemoragik. Pelaksanaan asuhan keperawatan meliputi tahap pengkajian, penetapan diagnosis keperawatan, penyusunan intervensi, implementasi, dan evaluasi yang dilakukan selama tiga hari. Hasil: Hasil pengkajian menunjukkan bahwa pasien mengalami gangguan mobilitas fisik. Setelah pelaksanaan asuhan keperawatan selama tiga hari, terjadi peningkatan kekuatan otot ekstremitas kanan dari skala 0 menjadi skala 2 yang disertai dengan peningkatan kemampuan mobilisasi pasien. Kesimpulan: Pelaksanaan asuhan keperawatan pada pasien stroke hemoragik dengan gangguan mobilitas fisik menunjukkan adanya peningkatan kekuatan otot dan kemampuan mobilisasi pasien. Kata Kunci: Asuhan Keperawatan, Gangguan Mobilitas Fisik, Stroke Hemoragik. Background: Hemorrhagic stroke can lead to impaired physical mobility, requiring nursing care to help improve the patient's ability to move. Objective: To describe the implementation of nursing care for a patient with hemorrhagic stroke and impaired physical mobility. Method: This applied research employed a nursing care approach for a patient with hemorrhagic stroke. The nursing care process included assessment, nursing diagnosis, intervention planning, implementation, and evaluation, which were carried out over a period of three days. Results: The assessment showed that the patient experienced impaired physical mobility. After three days of nursing care, the patient's right extremity muscle strength improved from grade 0 to grade 2, accompanied by improved mobility. Conclusion: The implementation of nursing care for a patient with hemorrhagic stroke and impaired physical mobility resulted in improved muscle strength and mobility.
