Abstract

Latar Belakang : Klien Intracerebral Hemorrhage (ICH) dengan penurunan kesadaran yang terpasang NGT memiliki risiko tinggi mengalami pneumonia aspirasi. Posisi head up 30° merupakan intervensi keperawatan untuk menekan risiko komplikasi tersebut. Tujuan : Mendeskripsikan penerapan posisi head up 30° pada klien ICH dengan risiko aspirasi pada klien Intracerebral Hemorrhage (ICH) dengan Nasogastric Tube (NGT) di ruang Stroke Center. dalam menurunkan risiko aspirasi dan memperbaiki bersihan jalan napas pada klien ICH dengan NGT. Metode : Studi kasus deskriptif terhadap dua klien ICH yang terpasang NGT di Stroke Center RSUD Ulin Banjarmasin pada tahun 2026. Data dikumpulkan melalui observasi klinis, wawancara, dan studi dokumentasi selama 3 hari intervensi. Hasil : selama 3 hari efektif memperbaiki parameter klinis kedua klien; frekuensi napas (RR) menurun dari 26x/menit menjadi 20x/menit (Tn. A) dan 18x/menit (Ny. K), serta saturasi oksigen (SpO2) meningkat dari 95-96% menjadi 99%. Akumulasi sekret (suara gurgling) berkurang drastis hingga suara napas bersih, dan tidak ditemukan adanya insidensi aspirasi selama pemberian nutrisi enteral. Kesimpulan : Asuhan keperawatan selama tiga hari melalui pemberian posisi head up 30° efektif mengatasi masalah bersihan jalan napas tidak efektif pada Tn. A dan Ny. K dengan diagnosa Intracerebral Hemorrhage (ICH). Intervensi ini terbukti mampu memperbaiki parameter respirasi, menstabilkan frekuensi napas dalam rentang normal, serta mencegah komplikasi aspirasi pada kedua klien. Oleh karena itu, tindakan ini direkomendasikan sebagai intervensi mandiri keperawatan yang konsisten di ruang Stroke Center. Kata Kunci : Stroke, ICH, Head Up 30°, NGT, Risiko Aspirasi, Asuhan Keperawatan Background : Patients with Intracerebral Hemorrhage (ICH) who experience a decreased level of consciousness and are fitted with a Nasogastric Tube (NGT) are at high risk for aspiration pneumonia. The 30° head up position for nursing intervention designed to prevent aspiration. Objective : To describe the application of a 30° head-up position in Intracerebral Hemorrhage (ICH) patients with a Nasogastric Tube (NGT) to reduce the risk of aspiration and improve airway clearance. Method : A descriptive case study involving two ICH patients fitted with an NGT at the Stroke Center of RSUD Ulin Banjarmasin. Results : The 30° head-up position for 3 days effectively improved clinical parameters in both patients; respiratory rate (RR) decreased from 26 bpm to 20 bpm (Mr. A) and 18 bpm (Mrs. K), while oxygen saturation (SpO2) increased from 95-96% to 99%. Secretion accumulation (gurgling) was significantly reduced to clear breath sounds, and no incidence of aspiration occurred during enteral feeding. Conclusion : Three days of nursing care utilizing 30-degree head-up positioning effectively resolved ineffective airway clearance in Mr. A and Mrs. K with Intracerebral Hemorrhage (ICH). This intervention proved capable of improving respiratory parameters, stabilizing the respiratory rate within the normal range, and preventing aspiration in both patients. Therefore, this procedure is highly recommended as a consistent independent nursing intervention in the Stroke Center unit. Keywords : Stroke ICH, Head-Up 30°, NGT, Aspiration Risk, Nursing Care.
