Abstract

Latar belakang: Intracerebral Hemorrhage (ICH) merupakan salah satu kondisi kegawatdaruratan neurologis akibat perdarahan spontan pada parenkim otak yang dapat menyebabkan peningkatan tekanan intrakranial, gangguan fungsi motorik, serta penurunan kesadaran. Hipertensi merupakan faktor risiko utama yang berkontribusi terhadap tingginya angka morbiditas dan mortalitas pada pasien ICH. Tujuan: Tujuan penelitian ini adalah untuk menggambarkan pelaksanaan asuhan keperawatan pada pasien dengan Intracerebral Hemorrhage (ICH) di Ruang Nusa Indah RSUD dr. Doris Sylvanus Palangka Raya. Metode : Metode penelitian menggunakan pendekatan deskriptif kualitatif dengan desain studi kasus pada dua pasien dengan diagnosis medis Intracerebral Hemorrhage (ICH). Teknik pengumpulan data meliputi obsevasi, wawancara, pemeriksaan fisik, dan studi dokumentasi rekam medis. Hasil: Diagnosis keperawatan yang ditegakkan meliputi penurunan kapasitas adaptif intrakranial, nyeri akut, dan risiko perfusi serebral tidak efektif dengan intervensi utama meliputi posisi head up 30°, teknik relaksasi napas dalam, serta kolaborasi pemberian manitol dan analgesik. Evaluasi setelah tiga hari perawatan menunjukkan skala nyeri dan stabilisasi hemodinamik pada kedua pasien. Kesimpulan: Asuhan keperawatan yang komprehensif dan berbasis bukti efektif dalam mencegah perburukan kondisi neurologis pada pasien Intracerebral Hemorrhage (ICH) Background: Intracerebral Hemorrhage (ICH) is a neurological emergency caused by spontaneous bleeding in the brain parenchyma, which can lead to increased intracranial pressure, motor dysfunction, and impaired consciousness. Hypertension is a major risk factor contributing to high morbidity and mortality rates in ICH patients. Objective: The objective of this study is to describe the implementation of nursing care for patients with Intracerebral Hemorrhage (ICH) in the Nusa Indah Ward of dr. Doris Sylvanus General Hospital, Palangka Raya. Method: The research method used a qualitative descriptive qualitative approach with a case study design involving two patients diagnosed with Intracerebral Hemorrhage (ICH). Data collection techniques included observation, interviews, physical examinations, and a review of medical records. Results: The established nursing diagnoses included decreased intracranial adaptive capacity, acute pain, and risk of ineffective cerebral perfusion. The primary interventions included a 30˚ head up position, deep breathing relaxation techniques, and the coordinated administration of mannitol and analgesics. An evaluation after three days of care showed improvement in pain scores and hemodynamic stabilization in both patients. Conclution: Comprehensive, evidence-based nursing care is effective in preventing the worsening of neurological conditions in patients with Intracerebral Hemorrhage (ICH).
