Abstract

Latar Belakang : Intracerebral Hemorrhage (ICH) atau stroke hemoragik diakibatkan oleh pecahnya pembuluh darah di otak. Kondisi tersebut menimbulkan gejala neurologis yang berlaku secara mendadak dan seringkali diikuti gejala nyeri kepala yang berat pada saat melakukan aktivitas akibat efek desak ruang atau peningkatan tekanan intrakranial. Pasien post-craniotomy memerlukan asuhan keperawatan intensif dan sistematis di ICU untuk memantau stabilitas neurologis. Tujuan : Menguraikan asuhan keperawatan pada pasien ICH Post Craniotomy di Ruang ICU RSUD Ulin Banjarmasin melalui proses keperawatan lengkap. Metode Penelitian : Penelitian ini menggunakan pendekatan studi kasus dengan teknik pengumpulan data melalui wawancara, observasi, dan studi dokumentasi. Hasil : Proses keperawatan telah dijalankan dengan runtut, meliputi pengkajian status neurologis, penentuan diagnosis keperawatan, penyusunan intervensi, pelaksanaan implementasi, serta evaluasi hasil secara berkelanjutan yang terdokumentasi dengan baik. Intervensi utama mencakup pemantauan neurologis, stabilisasi fungsi vital, serta pencegahan komplikasi yang terbukti memberikan dampak positif terhadap perbaikan kondisi klinis pasien di ruang intensif. Saran : Perawat diharapkan mampu mengoptimalkan pemberian asuhan keperawatan serta melibatkan keluarga secara aktif dalam mendukung proses pemulihan pasien dengan Intracerebral Hemorrhage (ICH). Kata Kunci : Intracerebral Hemorrhage, Asuhan Keperawatan, Resiko Pefusi Serebral Tidak Efektik. Background: intracerebral hemorrhage (ich), or hemorrhagic stroke, is caused by the rupture of blood vessels in the brain. This condition results in sudden neurological symptoms and is often accompanied by severe headache during physical activity due to the mass effect or increased intracranial pressure. Post-craniotomy patients require intensive and systematic nursing care in the intensive care unit (icu) to monitor neurological stability. Objective: to describe the nursing care provided to a patient with post-craniotomy intracerebral hemorrhage in the icu of ulin regional general hospital, banjarmasin, through a comprehensive nursing process. Research method: this study employed a case study approach, with data collected through interviews, observations, and documentation review. Results: the nursing process was carried out systematically, including neurological assessment, nursing diagnosis formulation, intervention planning, implementation of nursing actions, and continuous evaluation of outcomes, all of which were well documented. The main interventions included neurological monitoring, stabilization of vital functions, and prevention of complications, which demonstrated positive effects on the improvement of the patient's clinical condition in the intensive care unit. Recommendation: Nurses are expected to optimize the provision of nursing care and actively involve the patient's family in supporting the recovery process of patients with Intracerebral Hemorrhage (ICH).
