Abstract

Latar Belakang : Epilepsi merupakan gangguan neurologis kronis yang ditandai dengan kejang berulang akibat aktivitas listrik abnormal pada otak. Perbedaan etiologi dan manifestasi klinis dapat menyebabkan perbedaan prioritas diagnosis keperawatan sehingga diperlukan asuhan keperawatan yang individual. Tujuan: Mendeskripsikan pelaksanaan asuhan keperawatan pada pasien dengan diagnosis medis epilepsi di RSUD Ulin Banjarmasin. Metode: Penelitian ini menggunakan desain studi kasus deskriptif pada dua pasien dengan diagnosis medis epilepsi selama 3×24 jam. Data dikumpulkan melalui wawancara, observasi, pemeriksaan fisik, dan studi dokumentasi, kemudian dianalisis secara deskriptif berdasarkan proses keperawatan. Hasil: Hasil pengkajian menunjukkan adanya perbedaan kondisi klinis pada kedua pasien. Tn. S dengan riwayat stroke iskemik memiliki prioritas diagnosis Penurunan Kapasitas Adaptif Intrakranial, sedangkan Tn. M dengan kejang berulang memiliki prioritas diagnosis Risiko Cedera. Intervensi keperawatan mengacu pada SDKI, SIKI, dan SLKI sesuai kondisi masing-masing pasien. Setelah implementasi selama 3×24 jam, pada Tn. S tidak ditemukan tanda perburukan neurologis selama masa observasi, sedangkan pada Tn. M tidak terjadi cedera selama masa perawatan meskipun sempat mengalami satu episode kejang. Kesimpulan: Asuhan keperawatan yang disesuaikan dengan kondisi klinis pasien mendukung pencegahan komplikasi dan meningkatkan keselamatan pasien selama perawatan. Background: Epilepsy is a chronic neurological disorder characterized by recurrent seizures resulting from abnormal electrical activity in the brain. Variations in etiology and clinical manifestations can lead to differences in nursing diagnosis priorities, necessitating individualized nursing care. Objective: To describe the implementation of nursing care for patients with a medical diagnosis of epilepsy at Ulin Regional General Hospital (RSUD), Banjarmasin. Methods: This study employed a descriptive case study design involving two patients diagnosed with epilepsy, monitored over a period of 3×24 hours. Data were collected through interviews, observations, physical examinations, and document reviews, then analyzed descriptively based on the nursing process. Results: Assessment results revealed differences in the clinical conditions of the two patients. Mr. S, with a history of ischemic stroke, had a priority nursing diagnosis of Decreased Intracranial Adaptive Capacity, whereas Mr. M, who experienced recurrent seizures, had a priority diagnosis of Risk for Injury. Nursing interventions were based on SDKI, SIKI, and SLKI standards, tailored to each patient's condition. Following the 3×24- hour implementation period, Mr. S showed no signs of neurological deterioration, while Mr. M sustained no injuries during the care period, despite experiencing a single seizure episode. Conclusion: Nursing care tailored to the patient's clinical condition supports the prevention of complications and enhances patient safety during treatment.
