Abstract
Latar belakang : Stroke hemoragik disebabkan oleh pendarahan ke dalam jaringan otak (haemoragia intraserebral) dan ruang sekitar otak. Kondisi ini menyebabkan penderita mengalami sakit kepala yang hebat secara tiba-tiba, penurunan tingkat kesadaran, peningkatan tekanan intracranial, dan mengalami defisit neurologis seperti hemiplegia, hemiparesis, afasia, dan apraksia. Tujuan : Mendeskripsikan Asuhan keperawatan pada Tn.M dan Tn.H dengan Diagnosa medis Stroke hemoragik di Ruang Alexandri 3 dan ICU RSUD dr. Moch. Ansari Saleh Kota Banjarmasin pada tanggal 30 Maret 2026 – 28 April 2026. Metode : Studi kasus melalui observasi, wawancara, analisis dokumen seperti rekam medis pasien, dan asuhan keperawatan. Hasil : Masalah keperawatan yang muncul pada kasus ini yaitu Risiko perfusi serebral tidak efektif (D.0017), nyeri akut D.0077), dan risiko infeksi (D.0142). Pada Diagnosis keperawatan utama, yaitu risiko perfusi serebral tidak efektif dengan hasil perfusi serebral pada pasien 1 meningkat dan pada pasien 2 menurun, dengan intervensi yaitu mengobservasi peningkatan TIK, pada pasien 1 mengalami keluhan sakit kepala tetapi tidak ada muntah, sedangkan pada pasien 2 mengalami muntah dengan penurunan kesadaran. Pada kedua pasien dilakukan posisi head up 30 derajat untuk menjaga kestabilan aliran darah dan oksigen ke otak. Saran : Pada kedua pasien dengan Diagnosis keperawatan utama risiko perfusi serebral tidak efektif pada pasien 1 intervensi teratasi, yaitu mengobservasi peningkatan TIK hasil membaik, karena keluhan sakit kepala menurun dan tidak ada muntah. Sedangkan pada pasien 2 intervensi belum teratasi, yaitu mengobservasi peningkatan TIK hasil menurun, karena tanda dan gejala pasien post OP craniotomy dengan penurunan kesadaran, ada muntah, dan riwayat hipertensi. Background: Hemorrhagic stroke is caused by bleeding into the brain tissue (intracerebral hemorrhage) and the spaces surrounding the brain. This condition causes patients to experience sudden severe headaches, decreased level of consciousness, increased intracranial pressure, and neurological deficits such as hemiplegia, hemiparesis, aphasia, and apraxia. Objective: To describe nursing care for Mr. M and Mr. H with a medical diagnosis of hemorrhagic stroke in Room Alexandri 3 and the ICU at Dr. Moch. Ansari Saleh General Hospital, Banjarmasin City, from March 30, 2026, to April 28, 2026. Method: A case study involving observation, interviews, and analysis of documents such as patient medical records and nursing care plans. Results: The nursing issues identified in this case were: Risk of ineffective cerebral perfusion (D.0017), acute pain (D.0077), and risk of infection (D.0142). Regarding the primary nursing diagnosis—risk of ineffective cerebral perfusion—cerebral perfusion improved in Patient 1 and decreased in Patient 2. The intervention involved monitoring increased intracranial pressure (ICP); Patient 1 reported headaches but no vomiting, while Patient 2 experienced vomiting accompanied by decreased consciousness. Both patients were placed in a 30-degree head-up position to maintain stable blood and oxygen flow to the brain. Suggestion: For both patients with the primary nursing diagnosis of risk of ineffective cerebral perfusion, the intervention was resolved in Patient 1—specifically, monitoring for increased ICP—with improved outcomes, as headache complaints decreased and there was no vomiting. Meanwhile, for Patient 2, the intervention was not resolved; specifically, the observed increase in ICP showed only partial improvement, as the patient exhibited signs and symptoms of post-craniotomy with decreased consciousness, vomiting, and a history of hypertension.
