Abstract

Background: Cerebral infarction occurs due to the death of brain tissue caused by the interruption of blood and oxygen supply, commonly triggered by a thrombus or embolus obstruction. This condition is a global health issue as it is a leading cause of death and disability. The increasing incidence is associated with risk factors such as hypertension, diabetes, and obesity. Objective: To manage nursing care for a patient with cerebral infarction, focusing on the risk of ineffective cerebral perfusion, acute pain, and the risk of falls. Methods: This study utilized a descriptive case study design with a three-day nursing care approach for Mrs. E, who experienced a cerebral infarction with a history of hypertension, cholesterol, and bruising injuries due to a fall. Data were collected through interviews, observations, physical examinations, documentation reviews, and literature studies. Nursing interventions focused on the management of increased ICP through a 30° head elevation, pain management through relaxation techniques and collaboration, and fall prevention through the installation of side rails and education. Results: After three days of nursing care, the problem of the risk of ineffective cerebral perfusion was resolved, characterized by the patient's stable consciousness (GCS 15) and vital signs within the safe range with a blood pressure of 135/85 mmHg. Additionally, the pain level successfully decreased from a scale of 6 to 4, and no recurrent fall incidents occurred during the patient's hospitalization. Conclusion: The combination of ICP management, pain management, and fall prevention interventions proved effective in resolving the primary nursing problems in cerebral infarction patients and preventing secondary injuries.
