Abstract

Latar Belakang: Stroke non-hemoragik merupakan gangguan neurologis yang terjadi akibat sumbatan pembuluh darah serebral sehingga menyebabkan penurunan perfusi otak dan berbagai defisit neurologis. Kondisi ini menjadi salah satu penyebab utama kecacatan yang berdampak pada penurunan kemampuan mobilitas fisik dan aktivitas sehari-hari pasien. Tujuan: Mendeskripsikan pelaksanaan asuhan keperawatan pada pasien stroke non-hemoragik di RSUD Dr. H. Moch. Ansari Saleh Banjarmasin. Metode: Penelitian ini menggunakan desain studi kasus dengan pendekatan proses keperawatan yang meliputi pengkajian, diagnosis, intervensi, implementasi, dan evaluasi keperawatan pada satu pasien dengan diagnosis stroke non-hemoragik. Hasil: Hasil pengkajian menunjukkan adanya kelemahan ekstremitas, penurunan kekuatan otot, dan gangguan komunikasi verbal. Diagnosis keperawatan yang ditegakkan meliputi gangguan mobilitas fisik, risiko perfusi serebral tidak efektif, nyeri akut, risiko jatuh, dan gangguan komunikasi verbal. Intervensi yang diberikan berupa latihan Range of Motion (ROM), manajemen nyeri, pencegahan jatuh, pemantauan perfusi serebral, serta dukungan komunikasi. Setelah dilakukan tindakan keperawatan, terjadi peningkatan kekuatan otot, perbaikan mobilitas fisik, penurunan keluhan nyeri, dan peningkatan kemampuan komunikasi. Kesimpulan: Asuhan keperawatan yang komprehensif dan berkelanjutan, khususnya melalui latihan ROM, efektif dalam meningkatkan mobilitas fisik dan mendukung proses pemulihan pasien stroke non-hemoragik. Background: Non-hemorrhagic stroke is a neurological disorder that occurs due to cerebral blood vessel blockage that causes a decrease in brain perfusion and various neurological deficits. This condition is one of the main causes of disability that has an impact on the decline of the patient's physical mobility and daily activities. Objective: To describe the implementation of nursing care for non-hemorrhagic stroke patients at Dr. H. Moch Hospital. Ansari Saleh Banjarmasin. Methods: This study used a case study design with a nursing process approach that included the study, diagnosis, intervention, implementation, and evaluation of nursing in one patient with a diagnosis of non-hemorrhagic stroke. Results: The results of the study showed the presence of weakness of the extremities, decreased muscle strength, and impaired verbal communication. Established nursing diagnoses include impaired physical mobility, risk of ineffective cerebral perfusion, acute pain, risk of falls, and impaired verbal communication. The interventions provided were in the form of Range of Motion (ROM) exercises, pain management, fall prevention, cerebral perfusion monitoring, and communication support. After nursing measures, there was an increase in muscle strength, improvement in physical mobility, a decrease in pain complaints, and an increase in communication skills. Conclusions: Comprehensive and continuous nursing care, particularly through ROM exercises, is effective in improving physical mobility and supporting the recovery process of non-hemorrhagic stroke patients.
