Abstract

Latar Belakang : SNH merupakan gangguan neurologis akibat penyumbatan pembuluh darah otak yang dapat menyebabkan gangguan perfusi serebral dan kelemahan anggota gerak sehingga menimbulkan masalah keperawatan. Studi kasus ini bertujuan menggambarkan pelaksanaan asuhan keperawatan pada pasien SNHdengan diagnosis keperawatan risiko perfusi serebral tidak efektif dan gangguan mobilitas fisik di Ruang Nusa Indah RSUD dr. Doris Sylvanus Palangka Raya. Metode Penelitian : Penelitian ini menggunakan metode deskriptif dengan pendekatan studi kasus pada Tn. L yang dirawat selama tiga hari. Pengumpulan data dilakukan melalui wawancara, observasi, pemeriksaan fisik, dan studi dokumentasi. Hasil Penelitian : Setelah dilakukan implementasi keperawatan berupa pemantauan status neurologis, tanda vital, tingkat kesadaran, latihan Range of Motion (ROM), perubahan posisi, mobilisasi bertahap, edukasi, dan kolaborasi terapi, diperoleh hasil bahwa masalah risiko perfusi serebral tidak efektif teratasi ditandai kesadaran compos mentis (GCS 15), tidak terdapat tanda peningkatan tekanan intrakranial, serta tanda vital stabil. Gangguan mobilitas fisik teratasi sebagian, ditandai pasien mampu mengikuti latihan ROM aktif terbantu dan menunjukkan toleransi aktivitas yang lebih baik, meskipun kekuatan otot ekstremitas kiri masih 3/3. Kesimpulan : Asuhan keperawatan mampu mempertahankan perfusi serebral dan meningkatkan kemampuan mobilisasi pasien secara bertahap, meskipun pemulihan kekuatan otot masih memerlukan perawatan lanjutan. Background : Non-hemorrhagic Strok is a neurological disorder caused by a blockage in a blood vessel in the brain, which can lead to impaired cerebral perfusion and weakness in the limbs, thereby presenting nursing challenges. This case study aims to describe the implementation of nursing care for a patient with a nonhemorrhagic Strok diagnosed with the nursing diagnoses of risk for ineffective cerebral perfusion and impaired physical mobility in the Nusa Indah Ward at Dr. Doris Sylvanus Regional General Hospital in Palangka Raya. Research Method : This study employed a descriptive method using a case study approach involving Mr. L, who was hospitalized for three days. Data were collected through interviews, observations, physical examinations, and a review of medical records. Research Results : Following the implementation of nursing interventions—including monitoring of neurological status, vital signs, level of consciousness, range of motion (ROM) exercises, repositioning, gradual mobilization, patient education, and therapeutic collaboration—the findings indicated that the risk of inadequate cerebral perfusion was effectively addressed, as evidenced by the patient being compos mentis (GCS 15), the absence of signs of increased intracranial pressure, and stable vital signs. The physical mobility impairment was partially resolved, as indicated by the patient’s ability to perform assisted active ROM exercises and demonstrate improved activity tolerance, although muscle strength in the left extremity remained at 3/3. Conclusion : Nursing care was able to maintain cerebral perfusion and gradually improve the patient’s mobility, although recovery of muscle strength still requires continued care.
