Abstract

Latar Belakang: Intracerebral Hemorrhage (ICH) merupakan subtipe stroke hemoragik dengan angka kematian mencapai 50% dalam 30 hari. Hipertensi menjadi penyebab utama perdarahan intraserebral yang memerlukan penanganan intensif di ruang ICU. Tujuan Penelitian: Menganalisis hasil pelaksanaan asuhan keperawatan pada pasien ICH meliputi pengkajian, diagnosis, intervensi, implementasi, dan evaluasi keperawatan. Metode Penelitian: Studi kasus deskriptif dengan subjek satu pasien ICH (Tn.S, 46 tahun) di ruang ICU RSUD dr. Doris Sylvanus Palangka Raya. Pengumpulan data melalui wawancara, observasi, pemeriksaan fisik, dan rekam medis. Hasil Penelitian: Pengkajian menunjukkan pasien dengan GCS E3V1M5 (somnolen), TD 169/96 mmHg, hemiparesis sisi kiri, dan perdarahan lobus temporal kiri 30 cc. Tiga diagnosis ditegakkan: Penurunan Kapasitas Adaptif Intrakranial (SDKI D.0066), Defisit Perawatan Diri (SDKI D.0109), dan Risiko Infeksi (SDKI D.0142). Intervensi mengacu pada SIKI meliputi manajemen peningkatan TIK, dukungan perawatan diri, dan pencegahan infeksi. Implementasi dilakukan sesuai rencana. Evaluasi menunjukkan ketiga masalah belum teratasi dalam 3 x 24 jam karena pasien masih dalam fase akut pasca operasi dengan edema perihematoma yang mencapai puncak pada hari ke-5 hingga ke-6. Kesimpulan: Asuhan keperawatan pada pasien ICH telah dilaksanakan secara sistematis sesuai standar, namun target pencapaian dalam 3 x 24 jam belum terpenuhi secara optimal karena keterbatasan waktu penelitian dan kondisi pasien yang masih dalam fase akut pasca operasi. Kata Kunci: Intracerebral Hemorrhage, Asuhan Keperawatan, Stroke Hemoragik, Tekanan Intrakranial Background: Intracerebral Hemorrhage (ICH) is a subtype of hemorrhagic stroke with a mortality rate reaching 50% within 30 days. Hypertension is the primary cause of intracerebral hemorrhage, requiring intensive management in the ICU. Objective: To analyze the implementation of nursing care in an ICH patient, including assessment, diagnosis, intervention, implementation, and evaluation. Method: A descriptive case study with one ICH patient (Mr. S, 46 years old) in the ICU of dr. Doris Sylvanus Regional Hospital, Palangka Raya. Data were collected through interviews, observation, physical examination, and medical records. Results: The assessment revealed a patient with a GCS of E3V1M5 (somnolent), blood pressure of 169/96 mmHg, left-sided hemiparesis, and a 30 cc hemorrhage in the left temporal lobe. Three nursing diagnoses were established: Decreased Intracranial Adaptive Capacity (SDKI D.0066), Self-Care Deficit (SDKI D.0109), and Risk of Infection (SDKI D.0142). Interventions were based on SIKI, including increased ICP management, self-care support, and infection prevention. Implementation was carried out according to the plan. The evaluation showed that none of the three problems were resolved within 3×24 hours because the patient was still in the acute postoperative phase with perihematoma edema peaking on days 5 to 6. Conclusion: Nursing care for the ICH patient was systematically implemented according to standards; however, the targeted outcomes within 3×24 hours were not optimally achieved due to the limited study timeframe and the patient's condition, which remained in the acute postoperative phase.
