Abstract

Latar Belakang: Intracerebral hemorrhage (ICH) merupakan stroke hemoragik dengan angka mortalitas dan morbiditas yang tinggi akibat perdarahan intraparenkim otak yang dapat menyebabkan edema serebral, peningkatan tekanan intrakranial, dan penurunan kesadaran. Kondisi ini memerlukan asuhan keperawatan yang komprehensif untuk mencegah komplikasi. Tujuan Penelitian: Menggambarkan pelaksanaan asuhan keperawatan pada pasien ICH di ruang ICU. Metode Penelitian: Penelitian ini menggunakan metode deskriptif dengan pendekatan proses keperawatan selama tiga hari pada satu pasien ICH. Data dikumpulkan melalui observasi, wawancara, pemeriksaan fisik, dan dokumentasi. Hasil Penelitian: Diagnosis keperawatan meliputi penurunan kapasitas adaptif intrakranial, bersihan jalan napas tidak efektif, gangguan mobilitas fisik, dan gangguan integritas kulit/jaringan. Intervensi dilakukan sesuai standar SDKI, SLKI, dan SIKI. Hasil evaluasi menunjukkan kondisi hemodinamik dan respirasi relatif stabil, tidak terjadi perburukan neurologis maupun luka tekan, namun tingkat kesadaran masih sopor sehingga masalah keperawatan belum sepenuhnya teratasi. Kesimpulan: Asuhan keperawatan yang sistematis membantu mempertahankan kondisi pasien, mencegah komplikasi, dan mendukung proses pemulihan. Kata Kunci: Intracerebral hemorrhage, Asuhan Keperawatan, Penurunan Kapasitas Adaptif Intrakranial, Jalan Napas, Mobilitas, Integritas Kulit Background: Intracerebral hemorrhage (ICH) is a hemorrhagic stroke with high mortality and morbidity rates caused by bleeding within the brain parenchyma, which can lead to cerebral edema, increased intracranial pressure, and decreased level of consciousness. This condition requires comprehensive nursing care to prevent complications. Objective: To describe the implementation of nursing care for patients with ICH in the ICU. Method: This study used a descriptive method with a nursing process approach over three days for one patient with ICH. Data were collected through observation, interviews, physical examinations, and documentation. Results: The nursing diagnoses included decreased adaptive intracranial capacity, ineffective airway clearance, impaired physical mobility, and impaired skin/tissue integrity. Nursing interventions were implemented based on the Indonesian Nursing Diagnosis Standards (SDKI), Indonesian Nursing Outcomes Standards (SLKI), and Indonesian Nursing Interventions Standards (SIKI). Evaluation showed that the patient's hemodynamic and respiratory conditions remained relatively stable, with no neurological deterioration or pressure injuries. However, the patient remained stuporous, indicating that the nursing problems had not been fully resolved. Conclusion: Systematic nursing care helped maintain the patient's condition, prevent complications, and support the recovery process.
